Insurance & Billing
We are providers for Delta, United Concordia, Assurant, Sunlife, Premera, Cigna, and Metlife. We will bill most insurances that allow you to come to the office of your choice. We don’t accept Apple Health or Willamette. Those two insurances require you to go to a specific office.
Our office accepts VISA, Mastercard, American Express and Discover credit cards. We also accept CareCredit, which is easy to apply for at www.carecredit.com or 1-800-365-8295. We also have an application here in the office and can usually determine your eligibility in minutes.
